Nominate a Caregiver

It takes a special person to come alongside families as their loved one comes to the closing of their life. Zwick & Jahn Funeral Homes is establishing an award to honor and create a spotlight as a way to recognize the selfless dedication shown to others by caregivers.

We are incredibly thankful for these individuals and the lasting compassion shown to others. When sharing a story, we will forward the story to that individual or organization. A sincere thank you goes far to brighten someone’s day and to show what they do is very meaningful. We will make every effort to share your kind words with the caregiver and their company.

Who is a Caregiver?

A caregiver is any individual or volunteer who provides care or support directly to a patient or their family. This can include a physician, nurse, chaplain, social worker, CNA, administrative professional, volunteer, or any individual who works directly with the patient or their family in Northeast Indiana. We ask that you nominate a caregiver that you believe exemplifies and shows selfless dedication to others. 

Each quarter we will honor one caregiver that has been nominated and present them with an award.
